Wildlife Admissions Form

Thank YOU for caring for Tennessee’s wildlife!!

While the asterisk items are the only ones required if you’re in a hurry, the more information you can provide the better able we’ll be able to help. The initial steps we take are informed by the data you provide and can be life-saving.

Please check all that pertain to what happened to the animal(s):*
Please check all that pertain to the animal(s) condition when found:*
If a baby, please select the description that most closely resembles the rescued animal(s).
See "START HERE" page for photos
Has the animal bitten or scratched anyone?
example: cleaned/treated wounds, used medications, etc
example: clear or yellow urine, solid or wet stools, black tarry stools or yellow stools or another color
Do you want to be contacted in the future about the animal’s welfare and potential release?

Notes: (1) Please understand that some animals may not recover from the traumatic event that precipitated the need for human assistance even if they seem fine at the time of rescue (e.g. internal bleeding). For example, if the mother was fatally injured in a car accident, some babies may have been injured as well and the injuries are not always immediately apparent. So please think carefully about your selection above. (2) Some animals may be returned to the place they were found (or nearby) to be released; some places or situations are not suitable so alternate locations may be necessary. If you think you have a suitable release location please let us know.

I understand that wildlife admitted to this wildlife rehabilitation facility become the sole responsibility of this facility, which is permitted by the state of Tennessee (TWRA) to provide care for wild animals with the intention to release them back into the wild in TN.*
